Arriving in the plainest of plain packaging (a clear plastic bag), without any instructions, you're left to figure it out on your own. In fairness, it's pretty simple to understand, inside the clear bag you'll find two small wrist cuffs and two large ankle cuffs.
The cuffs are made of leather, fully equipped with a tiny carabiner that reads, "rated 10kg”—plus two nylon straps that go around your bed.
You'll need to thread the end of these nylon straps into the buckles yourself. Looking at the flat, untextured side of the buckle, bring the end of the nylon strap up from underneath, on the innermost gap in the buckle. Then thread it back down through the outer gap. You should now be able to pull to tighten it.

To get technical for a moment, the wrist cuffs adjust from 5-9 inches circumference in 9 adjustment holes, while the ankle cuffs can go 8-14 inches with 13 adjustment holes. That’s sure to cover most body sizes, and
the sales page says the straps adjust to any double size bed, though my tiny British home won’t accommodate anything more than a standard Queen. There’s plenty to spare though, so I suspect it would handle a King size, but can’t confirm. You might need some DIY adjustments to make it fit a single bed.

Ease of use is definitely one of the best parts of the DOMINIX Under Mattress restraint system, but you'll still need to wrap it around your mattress somehow, either by throwing one end under the bed and grabbing it from the other side, or lifting the mattress to thread it under. Since the upper and lower straps are separate, it’s easy to lift a portion of the mattress rather than the whole thing.
After tightening it up, I had about a meter and a half of loose nylon webbing left, which I tucked under the bed. It’s a very neat setup.

Each cuff has two possible attachment points on the bed straps, depending on how much you want your captive's arms or legs to be spread apart.

Both the top and bottom straps are secured independently, so you can also adjust those on your mattress for a harder or softer restraint. Instead of having your arms by your head, you might want to try bringing them down to waist level, or perhaps lifting up the ankle cuffs so the legs are forced to bend.
If they’re flexible, you could even try a single strap in the middle, bringing their legs up and arms down.

The leather cuffs are incredibly comfortable, even for extended wear. You could argue that bondage isn't supposed to be comfortable, but for most people, it should be if you want to enjoy (and endure) it long enough to orgasm. If, like me, you're not aiming to actually torture your lover, a comfortable restraint system is a necessity.

The tiny carabiners are quite dainty and look like they'd be easy to remove, but in practice they're too fiddly for your sub to contort their hands and unclip themselves. It's not impossible and the determined sub will definitely be able to escape given enough time. You could swap them out for small padlocks easily enough.
My little minx decided it would be easier to twist the nylon strap around the bed frame, then use her teeth to unbuckle one of the cuffs. You could probably avoid this by wrapping the strap around one of the bed frame slats underneath.

Still, for most purposes, these are a lot more secure than I was expecting, given that most bedroom bondage gear tends toward being a toy. If you see the words “velcro” anywhere, you should run far away from that product.
The nylon bed straps, when tightened properly, have surprisingly little give. Your sub will be able to squirm a little, but overall they'll stay put.
